Elder Care Access also provides the opportunity to consult with a professional geriatric care manager for an in-home evaluation to develop a care plan specific to your needs and wishes, as well as your elder’s unique needs.

Sheryl Fappiano is a professional Geriatric Care Manager (GCM) and licensed social worker with over 35 years of experience working with seniors in various capacities. She works ​closely with local guardians, families, protective services, and home care agencies to guarantee that your loved one is safe and comfortable.
