LPN/Case Manager

CNA/HHA/Companion

ECA is seeking compassionate and dedicated CNAs, HHAs, and Companions to join our team and work with the elderly population in their homes. If you have a passion for caregiving and a proven track record of supporting seniors, we’d love to hear from you!

​Position Details:

  • Conduct patient assessments and create individualized care plans for senior patients.
    Coordinate managed care services while ensuring HIPAA compliance.

  •  Facilitate discharge planning and transitions, offering education and support to patients and families, and care givers

  • Collaborate with interdisciplinary teams to optimize outcomes across care settings.

  • Monitor and update care plans based on patient progress and changing health needs.

  • Offer assistance and options for housing, downsizing and for appropriate care as Financial and functional needs change

  • Assist in moving clients to another facility

Qualifications:

  • Experience working with elders (a must!)

  • Reliable transportation

  • Willingness to be on-call

Compensation & Benefits:

  • 20-30 hrs a week

  • Flexible schedule M - f 8am-6pm

Position Details:

  • Provide assistance with Activities of Daily Living (ADLs), light housekeeping, and companionship.

  • Accompany clients on walks, outings, or appointments.

  • Run errands and provide transportation as needed.

  • A few clients enjoy calm dog visits or pet therapy (if applicable to the caregiver).

  • Flexible Schedule: 24-32 hours per week, with shifts lasting 2-4 hours.

  • Availability for every other weekend shifts as needed.

 

Qualifications:

  • Minimum 2 years of experience working with seniors, including individuals with dementia.

  • Reliable car and a valid driver’s license are required.

  • A genuine passion for improving the quality of life for elderly clients.

  • Not Mandatory; We have a few clients that love visits from dogs if they are vaccinated and do no jump or bite!
